Permission Slip form for: Class Representative

You have been elected by your class and teacher to be ________________'s SC Class Representative!

Requirements for SC Rep
1. I will attend the SC kick off meeting with a parent on Sept 2nd from 6:00-7:00pm in the library.
2. A parent and I must work at 1 student store and at Hoe Down or Spring Fling
a. Student stores are one Friday a month from 10am to 2pm
3. I must be on one student council committee. (these will be signed up for at the meeting on September 2nd )
4. I must be willing to work on any other activities or events that student council votes on doing.
5. I will attend Student Council Meetings (you will be replaced after the second absence)
a. Meetings are on one Wednesday of each month from 3:15-4:00 (see the calendar)
6. I will take notes and report SC business to your class.
7. I must be organized with excellent study skills.
8. I must not have any report card grade below a C.
